3. Instructions
Form Filling Instructions
- First you need to complete your profile by giving your details. Keep a clear, visible digital copy of passport size photo of yours ready to upload. This photo will be used for creating your ID Card.
- In the application form, please write your full name as per your official records such as Aadhaar card or passport. Prefered format of Full Name is "FirstName MiddleName LastName".
- After filling up the application form and submission, your membership profile (or application) will be created. Then please verify your details in the saved profile and make any corrections by clicking Edit button. You will be able to edit your full profile details until your membership is granted by our office. But once your membership is granted, you will NOT be able to edit few fields. They are your name, profile picture, age, gender, address and nationality. But all other details can be edited at any time even after becoming member. Once you feel all filled-in information is correct, then proceed for making/submitting membership payment.
Payment Instructions
You have to make payment externally by any one method mentioned below. Then submit the payment details in this form. You need to upload image of proof of payment.-
You can pay by any one of UPI method [Directly to JLM Account]-
Bhim / UPI / Google Pay / PhonePe / PayTM etc., to our UPI id:
JLMAHASABHA@SBI OR scan below QR code to pay.

You can make direct NEFT / IMPS / RTGS payment [Directly to JLM Account] to our bank account from your bank account online. Our Bank details are as below:

You can make payment by Cheque or DD [Directly to JLM Account]
It should be payable to "Jagatika
Lingayat Mahsabha" or "ಜಾಗತಿಕ ಲಿಂಗಾಯತ ಮಹಾಸಭಾ" and should be crossed.
Ensure no mistake and no over writings. If your bank account is within
Karnataka, you may write your cheque in Kannada also as per RBI guideline.
You need to send it to our office by speed post or courier. Do not forgot
cross it and to write your name & mobile number at the back of cheque or DD.
